Operative portography demonstrates a single extrahepatic porto-systemic shunt. The splenic vein is cannulated and contrast medium is seen to pass from the portal venous system in the caudal vena cava, bypassing the liver.

The catheter is positioned in the mesenteric vein; contrast media outlines the mesenteric vein, portal vein, shunt (arrowhead), and caudal vena cava. The cat was a 1-year-old male domestic shorthair cat with signs of stunted growth, aggressive behaviour changes, and urate urolithiasis.
